For someone to send UNT Health Clinical Practice Group your records, please complete the Authorization To Receive Protected Health Information (English)(Spanish) and deliver to the doctor’s office or provider you are requesting release documents to us. They will then be able to send over your records.

No, it is free to request your medical records, however there may be fees associated with legal requests.
To request a change to your record, contact us at 817-735-2185 and you may be asked to complete a Medical Record Amendment Request form. Understand that the provider is not required to make your requested change, but will review your request and respond in accordance with applicable laws.
Yes, you can schedule an appointment with an HIM specialist. Our staff must remain with the medical record and with you while you review the information.
